While the causes of Americas sleep woes are up for debate, theres little disagreement about Americas favorite remedy: Melatonin, the countrys most-used sleep aid.

Melatonin Use Linked To Lower Risk For Vancomycin

Melatonin, a commonly used sleep aid, may significantly reduce the risk for acute kidney injury in patients treated with vancomycin, according to a new observational study published in Antimicrobial Agents and Chemotherapy.

In a retrospective cohort study at a large community medical center, researchers examined 303 hospitalized adult patients treated with vancomycin from January 2016 to September 2020. After controlling for potential confounders, melatonin use was significantly associated with 63% decreased odds of AKI.

The cohort consisted of 101 patients treated with melatonin as part of ordinary care and 202 control patients. The melatonin and control groups had a mean age of 60.9 and 64.4 years, respectively. The primary endpoint was development of AKI, defined as an absolute increase in serum creatinine of 0.3 mg/dL or higher or a 50% or greater increase in serum creatinine level. Both groups had similar characteristics, but the treatment group had a significantly higher incidence of bacteremia/sepsis .

We also suspect that melatonin alleviates mitochondrial stress. There are data that suggest damaged mitochondrial function in early AKI. Perhaps melatonin functions to alleviate oxidative stress and restore mitochondrial balance, Dr Brunetti said.

On univariate analysis, concomitant use of piperacillin-tazobactam and loop diuretics was significantly associated with 3.9-fold and 3.4-fold increased odds of AKI, respectively.

âItâs generally safe, itâs not habit forming, and people donât really develop a tolerance for it,â Canapari said. âIt is my first-path sleep medication for children who need it.â

But other researchers say theyâre concerned about its safety.

In 2015, researchers from Australia published a study pointing to research that linked melatonin use on rodents to changes in when they start puberty. The report also notes that in the early 1990s, scientists were looking into high doses of melatonin as possible human birth control because of the way it could affect the reproductive system.

âConsidering the small advances melatonin provides to the timing of sleep, and considering what we know about how melatonin works in the body, it is not worth the risk to child and adolescent safety,â warned lead author David Kennaway, head of the Circadian Physiology Laboratory at the University of Adelaideâs Robinson Research Institute.

As expected, the remnant kidneys in the untreated rats showed significant glomerulosclerosis and tubulointerstitial damage. Melatonin administration significantly reduced the severity of glomerulosclerosis and tubulointerstitial damage. As shown in , glomerulosclerosis and the extension of tubular injury were reduced by approximately one-half in the melatonin-treated group.

A study by researchers at the Rutgers Ernest Mario School of Pharmacy finds that melatonin significantly reduces kidney failure in people being treated with the antibiotic vancomycin.

The study, in the journal Antimicrobial Agents and Chemotherapy, examined 303 hospital patients being treated with vancomycin and found that 101 who also received melatonin had a 63 percent decrease in acute kidney injury.

Globally, antibiotic use is one of the leading causes of drug-induced kidney failure, with more than 13.3 million cases reported each year.

Kidney failure is also associated with an increased risk of death in hospitalized patients. Almost 25 percent of these patients die in hospitals when toxins build up as they also receive antibiotic treatments for various infections.

Weve known from previous studies the ability of vancomycin to induce kidney injury through oxidative stress and mitochondrial damage, and we also knew how melatonin could restore renal mitochondrial function, so making these findings in this small group of patients was promising and could lead to bigger therapeutic breakthroughs, said the studys lead author, Luigi Brunetti, an associate professor at the Rutgers Ernest Mario School of Pharmacy.

The study was the first to examine such human data, and researchers plan further clinical studies on a larger and broader population.

We hope our findings pave the way for larger group studies as acute kidney injury affects millions of people, Brunetti said.
