Genetic variants can be divided into two broad classes, based on their frequency and their functional effects on the target gene. Polymorphisms are common genetic variants that occur frequently in the population and generally have only very modest effects on gene function. In contrast, mutations are rare genetic variants with major effects on gene function leading to monogenic disorders such as RTH, RTH , or the Allan-Herndon-Dudley syndrome due to mutations in the TH transporter MCT8 . While no patients have been identified with functionally important deiodinase mutations, seven patients with mutations in SECISBP2, a gene critical for the synthesis of selenocysteine containing proteins including the deiodinases, have been identified. Affected individuals have a mild phenotype but do exhibit decreased serum T3 levels .

Approach Utilized In Conducting This Review

Our task force was commissioned and approved by officers of the ATA. We formed in the summer of 2011 and our composition included members with particular expertise in mechanistic and translational science , members with particular expertise in clinical thyroidology , a member with a background in pediatric endocrinology, a member with expertise in design and evaluation of health research, and a bioethicist. As of October 2011, we were redirected and specifically asked to develop clinical practice guidelines. Our task force met face to face in October 2011, June 2012, September 2012, June 2013, October 2013, and June 2014 with seven interim discussions using conference calls. A spring meeting of the ATA that was open to all potential stakeholders was held on April 25 and 26, 2013, to highlight and promote broad discussion regarding relevant and topical issues . Information needs identified by stakeholders at this meeting were incorporated in task force group decisions in considering topics for review.

Role Of Srcs In Other Rth Models

Inability to recruit co-activators by mutant THRB is thought to play a role in the development of RTH. In ThrbPV mutant mice, a model of RTH with a knock-in mutation in the Thrb locus that completely abolishes T3 binding to THRB , lack of Src1 leads to a worsening of the HPT axis dysregulation and growth abnormalities in ThrbPV/+ mice but not in ThrbPV/PV mice. In TrbPV/PV animals, Src1 deficiency intensified the pathological progression of thyroid follicular cells to papillary hyperplasia. Lack of Src1 did not affect serum cholesterol levels in either ThrbPV/+ or ThrbPV/PV mice, but led to dysregulated expression of several T3 target genes in the pituitary and liver .

BOSTON – There’s a newcomer to the diet scene. It’s called the Hormone Reset Diet and it promises to help women lose weight, but Dr. Mallika Marshall says before you sign on, there are some things you should know.

This new method was conceived by a doctor, a gynecologist in California. She claims women can lose 15 pounds in 21 days if they follow seven dietary guidelines, each designed to address a specific hormonal imbalance.

For example, for estrogen, go meatless and eliminate alcohol. Get rid of fruit to reset the hunger hormone, leptin. Toss out grains to reset your thyroid, and remove dairy to address your growth hormone levels.

You’re supposed to make one of seven dietary changes every three days.

But Boston University registered dietician and nutrition professor, Joan Salge Blake, says with this hormone reset diet, there are some real red flags.

“There’s no literature to suggest that you can by cutting certain foods out of your diet reset your hormones,” says Salge Blake. “Nor do you have to reset your hormones for you to lose weight. Another gut check? Be careful when you see weight loss plans that try to eliminate whole food groups. That’s usually a flag saying why does a whole food group need to be eliminated when you’re trying to lose weight?”

Salge Blake adds that there is really no science to back up this diet, and even though you will probably lose weight, you’re unlikely to be able to keep the weight off.

SmrtmRID mouse model bears mutations in two Smrt RIDs , which abolish its interactions with RAR, TR, and PPAR isoforms . SmrtmRID mice are born at expected ratios, but have slightly reduced viability on a mixed C57BL/6-Sv129 genetic background. Adult animals display a number of metabolic disturbances: significantly increased adiposity with reduced body weight, decreased EE, and RER, increased basal hepatic glucose production, and diminished insulin-stimulated glucose disposal leading to elevated fasting glucose and glucose intolerance. MEFs isolated from SmrtmRID mice demonstrate increased PPARG signaling and adipogenic potential suggesting that SMRT, by repressing signaling by PPARG and potentially other NRs, plays an important role in determining the adipogenic set point. Hypothyroid SmrtmRID mice demonstrate significant derepression of some TH target genes in the liver and improvement of hypothyroidism-induced hypercholesterolemia, indicating that SMRT plays a role in repression of positive TR targets in the absence of hormone. This is contrary to what was seen in the liver-specific Smrt knockout , and whether the differences are due to the whole-body vs liver-specific deletion or the ability of mutant SmrtmRID to still bind HDAC3 and other components of the repression machinery remains to be clarified.

In general, levothyroxine should be initiated with low doses, and the dose titrated slowly based on serum thyrotropin measurements. It should be recognized that normal serum thyrotropin ranges are higher in older populations , and that higher serum thyrotropin targets may be appropriate.

In elderly persons who are without known heart disease or without major risk factors for heart disease, LT4 therapy can be initiated at the full dose , although the method of starting with a low dose and increasing it slowly is still preferred by some experts . The final LT4 dose that normalizes the serum TSH is generally lower in the elderly compared to younger persons , related to decreases in T4 turnover with age, in turn caused by decreases in lean body mass . However, other factors in the elderly, such as decreased thyroid hormone absorption concomitant drug use, which is more common in the elderly and other comorbidities may serve to negate the effects of decreased T4 metabolism, leading to an LT4 dose that is closer to what would be predicted in a younger person.

There are no adjustments in LT4 dosing that are required in cases of cirrhosis or renal failure. Nephrotic syndrome, with its large urinary protein losses that include the thyroid hormone transport proteins TBG, transthyretine and albumin, can be a cause of increased LT4 requirements due to excessive urinary thyroid hormone losses .

Triiodothyronine Monotherapy For Hypothyroidism

14.Are there data regarding therapy with triiodothyronine alone, either as standard liothyronine or as sustained release triiodothyronine, that support the use of triiodothyronine therapy alone for the treatment of hypothyroidism?


Although short-term outcome data in hypothyroid patients suggest that thrice-daily synthetic liothyronine may be associated with beneficial effects on parameters such as weight and lipids, longer-term controlled clinical trials using a longer-acting form of triiodothyronine are needed before considering the endorsement of synthetic liothyronine therapy for routine clinical use.

Although an extended-release formulation of LT3 has been shown in a single short-term study to reduce the peaks and troughs of LT3 administration that were seen over a 9-hour period after T3 administration , currently no formal pharmacokinetic study has been published. Presumably, once in the circulation as T3, the persistence of T3 is limited by its short half-life. Thus, even though a sustained release preparation may have reduced peaks and troughs, it may still need to be administered several times day. However, interestingly, a report of a sustained release profile for serum T3 levels after administration of a single dose of T3 sulfate to hypothyroid patients was recently published .
